Complete and sign your onboarding documents below. This form is required before your first day. All information is encrypted and stored securely through Adobe Sign.

Instructions — Please Read Before Starting

1. Fill out every section of the form completely. Required fields are marked with an asterisk (*).
2. Have your driver's license and Social Security Number ready before you begin.
3. For the W-9 section, select the tax classification that applies to you. If you are an individual or sole proprietor, select that option and enter your SSN.
4. For payment, choose Direct Deposit, CashApp, or Venmo and fill in the corresponding details.
5. Provide at least one emergency contact with a working phone number.
6. When finished, sign the form using your mouse, finger, or by typing your name.
7. Click Submit — you will receive a copy by email and Samantha will be notified automatically.
